DOCUMENTS REQUIRED :-

Filled in Interview Particular annexed with signature of candidate and four additional passport size color photographs.
MCI/State Medical Council Permanent Registration Certificate (compulsory).
Pass certificate andmark sheets (all years) for Educational and Professional Qualifications.
Certificates showing relevant post qualification experience.
Proof of Date of Birth (Matriculation Certificate).
NOC from present employer, if already employed in Govt./semi-Govt./Public Sector Undertaking.
Caste Certificate for SC/ST/OBC (non-creamy layer),as applicable.
Disability Certificate for PH candidates (As applicable).
